from the Historical Harp Society!
This two-page order form and instruction sheet helps us 
to process your order correctly.

There are two ways that you may order:
* ELECTRONICALLY:
1. mark your order on this form by placing XX next to the price of the piece that you want to order (use the .rtf or .doc version form)
2. write your contact information, including the e-mail address you use in PayPal, at the bottom of this page
3. save the revised form (both pages)
4. e-mail it back to QMAH@aol.com
5. use PayPal to send the correct payment, using the e-mail address hhstreas@yahoo.com
* HARD COPY
1. print both pages of this form and circle what you would like to order (use any version of the form)
2. add your contact information to the bottom of this page
3. mail the form back with a check for the proper amount to:
